The MPAR program was launched on September 1st, 2011.
MPAR is a survey-based assessment program.
There are currently 9 versions of MPAR. Each version uses surveys that have been designed and modified to suit the unique needs of each of the 9 categories of physicians.
At present, there are certain practice areas for which there is no MPAR review available. These include administrators, educators, or researchers not engaged in clinical practice or whose patient volume is too small to provide the information required to assess that physician’s performance.
To view the samples surveys for each of the 9 versions of MPAR, click on the version you wish to view on the left. Each version will include the following surveys:
- Self-assessment questionnaire
- Medical Colleague questionnaire
- Co-worker questionnaire
- Patient questionnaire or referring physician questionnaire
